| SIGNALMENT:
13 year old male mix-breed dog
HISTORY:
A chronic cough is reported by the owner. Two years ago, a diagnosis
of congestive heart failure was made and the dog started on enalpril.
Chest films taken at that time showed cardiac enlargement (not shown).
Despite enalpril and occasional furosemide therapy, the cough has persisted.
DIAGNOSTICS:
EKG and thoracic radiographs were done. The EKG shows cardiac
enlargement and atrial fibrillation. An ultrasound of the heart shows
cardiac enlargement and mitral valve regurgitation. Chest films show
a mass in the left caudal lung lobe in addition to pulmonary changes compatible
with heart disease. (Figures 1 and 2)
SURGERY:
A left 6th intercostal thoracotomy was done and a lobectomy of the affected
caudal lung lobe done. The mass was noted to be circumscribed, 7-8
cm. in diameter, and firm. On evaluation for histopathology, the
center of the lesion is cavitated and filled with purulent material.
Histopathology and culture samples were submitted. Local lymph nodes
were not enlarged. |
