 CASE 40

SIGNALMENT:

4 year old male Yorkie

HISTORY:

A one month history was given of the owner noticing a mass protruding from the oral cavity, located adjacent to the right side of the base of the tongue. Treatment with antibiotics and anti-inflammatory medication for 3 weeks had not altered the appearance of the mass.

DIAGNOSIS:

Oral examination shows what appears to be a dilatation of the terminal end of the mandibular and sublingual salivary gland duct on the right. (Figures 1 and 2) The mandibular duct was catheterized and 0.5 cc's of iodinated contrast material injected. Dye pools in the dilated region of the duct but does not appear to enter the proximal salivary duct. No sialoliths are noted radiographically and none can be palpated. (Figure 3)

Figure 2
Close-up view. The papilla and mandibular duct orifice can be appreciated.

COMMENT:

This appears to be an unusual presentation of a salivary mucocele, or ranula, possibly associated with trauma to the duct. The treatment recommended is excision of the mandibular and sublingual salivary glands and excision of the ranula with histopathology on excised tissues.

Contrast material in the dilated portion of the mandibular salivary duct (sialogram). Dye cannot be seen in the duct coursing towards the gland.
FOLLOW-UP:

The owner reported that the enlarged duct had dramatically diminished in size without further treatment by 2 1/2 months after initial examination.

ACKNOWLEDGMENT:

Thank you to Dr. David Merkey, Avalon Veterinary Hospital, for this referral.
